BIOMECHANICS OF THE VERTEBRAL COLUMN IN SPORTS. MECHANICAL LOW BACK GAINS
Summary
A bibliographic review of the biomechanics of the main sports vertebral injuries observed in athletes in the technical training programs of Catalonia and the ARC program (High Performance of Catalonia) is carried out. Different articles and books on sports medicine have been reviewed, avoiding introducing purely traumatological pathologies that would escape the main objectives of this work. We try to classify spinal pathologies from a point of view of the biomechanical origin of the injury through clear and concise explanations, accompanied by appropriate iconography.
Sports medicine would be the equivalent of the athlete's family medicine and what we aim to do is provide orthopedic concepts of the main pathologies of the athlete's spine that a podiatrist may encounter in his daily practice. We totally avoid pathology of the spinal column due to trauma.
The concepts that we introduce in a very generalized way always follow the same scheme: description of the pathology, etiology, clinic, diagnosis and generic treatment. It is mentioned which sports are most susceptible to producing spinal injuries, as well as their biomechanical mechanism of production.

BIOMECHANICS OF THE VERTEBRAL COLUMN IN SPORTS. MECHANICAL LOW BACK GAINS
Introduction
Technical or elite sport forces the spine, either intermittently or continuously, to adopt non-physiological positions or movements. This article includes a bibliographic review of the main pathologies presented by athletes who participate in the Tecnificación de Catalunya and ARC (High Performance of Catalonia) programs.
Non-physiological positions and movements produce two types of well-differentiated pathology that we will explain below:
– pathology derived from non-physiological positions maintained.
– pathology derived from sustained non-physiological movements.

Non-physiological positions facilitate vicious attitudes in athletes that, if they persist, produce structured static modifications. This occurs in childhood and adolescence when the bone is immature in both its structure and composition and therefore malleable. This phenomenon depends on the modified Delpech law.
We can talk about vicious attitudes that evolve into static modifications with respect to a frontal plane (scoliosis or scoliotic attitude) or sagittal plane (Scheüermann's disease and lumbar hyperlordosis).
A. Static frontal alterations.
– Structured scoliosis is a three-dimensional deformity that can be summarized as a torsion about its longitudinal axis. There are three types:
* congenital: from birth there are hemivertebrae or cuneiform vertebrae that cause scoliosis. Bad prognosis. It is currently believed to be not a primary bone and joint problem, but rather a secondary defect of dysfunction of the balance mechanisms of the brain and brainstem and the body's response to this dysfunction.
* unknown, idiopathic or essential cause: there is rotation of vertebral bodies (basic difference compared to those of the third group). The presence of curves greater than 10º is observed in the frontal plane with the risk of increasing as the patient grows. It is classified according to the age of the patient: infantile (under 3 years old), juvenile (from 3 to 10 years old) or until the beginning of adolescence and adolescent (from 10 years old or after the beginning of adolescence). Like the first type, it also has a poor prognosis.

* of known cause: they are those that can be related to sports, despite the fact that there is always a prior scoliotic attitude; Many times we will not be able to find out why they happen, but if we investigate a little they are always secondary to something.
– Regarding the scoliotic attitude, we know that it can be secondary to many causes, but in sports, the ones we see most frequently are due to lower extremity dysmetria and clinical crisis due to spondylolysis or spondylolisthesis:
* Lower extremity dysmetria: el patrón de la actitud escoliótica que se relaciona con una dismetría es siempre el mismo. No hay rotación de cuerpos vertebrales, existe curva lumbar convexa hacia el lado corto y curva dorsal convexa hacia el lado largo. Como podólogos podemos ayudar a confirmar el diagnóstico con la plataforma de presiones, ya que observaremos una alteración de cargas entre pies >10% y una separación entre el centro de gravedad del centro del cuerpo y el centro de presiones de cada pie muy diferente uno de otro. Con la ayuda de un escoliómetro situado en las gibas cuando el paciente hace una flexión dorsal encima de la plataforma de presiones, podemos calcular la medida del alza que produce un mejor apoyo plantar y una reducción de gibas. La colocación de una cuña, habitualmente en la mitad de la totalidad de la dismetría, suele ser suficiente para corregir en parte la actitud.
* Clinical crisis due to spondylolisis or spondylolistesis: The lesions of the interarticularis of the vertebra are frequent in the sport and appear in times of growth that coincide with sports technification. During the appearance of it there may be a contracture of lumbar or dorsolumbar muscle mass that produces this attitude. If it is maintained, it can be structured.
B. Sagittal static alterations.
- Scheüermann's disease es una hipercifosis dorsal (> 45º) rígida que sigue hacia la pubertad con acuñamiento anterior de al menos 3 vértebras consecutivas. En el estudio radiográfico se observa la deformidad cuneiforme de las vértebras y los nódulos de Schmorl (muescas profundas en las caras superiores e inferiores de las vértebras adyacentes) y a veces osteofitosis. Ocurre durante el pico de crecimiento puberal y es más frecuente en niños que en niñas. La causa más probable es la necrosis ósea aséptica con disminución de la irrigación sanguínea en la zona de crecimiento. Existe cierta base genética. A pesar de ser una alteración que encontramos en la población no deportiva, la práctica del deporte incrementa la aparición del dolor. Se relaciona con el síndrome de los isquiotibiales cortos y con posiciones viciosas mantenidas, como la posición en flexión de la columna vertebral al estar sentado en sillas pequeñas.

- Hyperlordosis lumbar. It is manifested by an exaggeration of lumbar lumbar in standing, with a sculpture of the pelvis forward, prominent abdomen and outgoing buttocks. It is more frequent in female sex. The commitment of the posterior arc of the vertebrae for this alteration produces facet or "mechanical pain." Mechanical pain is often understood the pain related to physical activity, although from an academic point the "mechanical pain" is the pain of the facetar syndrome, also known as the posterior branch.
Facting or mechanical pain syndrome is the most characteristic pain of sports. It can be secondary to lumbar hyperlordosis, discgeneration disc, anomalies of articular facets, dymetriah or instabilities.
The type of mechanical pain is one that increases in certain movements: getting up from a chair, performing a tennis service or arming to launch a ball or any sports object. It is a low, digital lumbar pain and with limited irradiation (frequently to buttocks, weird to the knee and never below the heel). In the exploration, anterior broadcasting is not observed.
There are certain positions of sport that favor a certain attitude and this a static modification. It seems that volleyball and tennis, for putting two examples, favor a certain tendency to cause a dorsal-convex scholaring attitude towards the active arm and a certain tendency to produce a dorsal kiphotic attitude. Contrary, fencing has a tendency to cause a dorsal-cócava scholarship attitude towards the armed arm and a certain tendency to produce a dorsal cupotic attitude. Despite all this, no one has shown that a sport produces structural alterations maintained in the spine.

The main mechanism of overload injuries are microtrauma. Within the spine we can talk about the following pathologies.
- Discgeneration. It causes a cracking in the fibrous ring of the intervertebral disc and its subsequent degeneration. Contrary to what you may think, sport does not increase the incidence of discourse, but discgeneration disc. The movements and forces that are involved in distal degeneration are compression, shear and rotations.
- Baastrup effect. It is produced by syndrome of the same name. It is an interspinous lower back secondary to some spinous megaapophysis that produce bone neo -articulations and apophysarian bursitis. Radiological bone bridges are established that produce interspinous osteoarthritis. This syndrome is incompatible with the daily realization of sport.
Relatively frequently, interspinous pain is observed without irradiation that occurs during repeated flexoextensions. When conducting a radiographic study, we observe some fat spinous apophysis, which produce neo -articulation. The GOP study is normal, but the SPECT demonstrates focal osteogenic activity at the expense of one or several spinous process, which translate a metabolitely active entholic enththic lesion in relation to the area of insertion of the supraespinous and interspinous ligament.
- Epifisitis lumbar. It is an aseptic osteonecrosis that is at the anterior angle of the vertebral body of the latest dorsal or first lumbar vertebrae. In lumbodorsal flexotione, intra -discharge pressure is mobilized back of the disc. In forced hyperextensions, intra -discal pressure goes to the anterior part of the disc. This pressure faces the powerful common intervertebral ligament and moves caudally to influence the growth area of the lower vertebra of the disc. This causes us a aseptic necrosis (epiphysitis). Sometimes, the athlete suffers nonspecific low back pain related to physical activity. If epiphysitis affects an important part of the vertebra, it is necessary to make functional radiological projections to show possible instability.
– Espondilólisis y Espondilolistesis: The spondyolysis in athletes represents a fracture of stress by repetitive loads with the clamping of the joint process of a segment against inter -articularis pars and the lower segment sheet with associated predisposing factors and a trigger. Spondylolysis describes a defect in the inter -articularis pars area. The main symptom of this pathology is a pain in the lumbar area that can radiate towards the buttocks or to the posterior face of the thighs and is frequently aggravated or reproduced with hyperextension and physical exercise. Isthymics sometimes does not radiate. If the sliding is serious, we can show in the palpation of the spine a step in the defect area. It is rare for children to have neurological clinic, if it occurs, it is because there is a marked sliding or is congenital. Sometimes, in the major landslides, the sign of Phalen-Dickson is observed that consists of a retraction of the hamstrings, posture in knee flexion and marches with the hips in flexion. L5 is usually affected. An incidence of 47% in teenage athletes has been found, so it is one of the most common causes of back pain at this age. The prevalence is about 4.4%, in 6 -year -old children, up to 6% in adults and occurs 2 times more in men than in women.
On the other hand, spondylolistesis is the displacement of a vertebra with respect to the immediate lower, equal to or greater than 2 mm both in the previous direction (anterolistesis, loss of height in the anterior part of the disc space) as posterior (retrolistesis, loss of disco loss of the disc in its back). Therefore, we can say that spondylolisis creates the occasion for a spondylolistesis to occur. It usually occurs in L5 regarding S1 and the incidence is practically the same in men as in women. Orthostatism submits to the Lumbo-Sacra Charnela to important mechanical loads that favor spondylolistesis.

Among the predisposing factors of spondylolis we have:
- Sex (more in men than in women)
- Age (from 6 years to 14 years)
- Raza (Etnia Inuit, Alaska)
- Inheritance (it is not congenital, but in 25% of the cases of spondylolysis there are a certain family association)
- Structure of pars
Among the trigger factors we have the sports overload of lumbar raquis, generally, lumbar hyperextension and rotation. Sports in which this lesion is more frequent is gymnastics, launches, liable, tennis, ballet, artistic skating and swimming (butterfly).
Hay varias razones por las que la espondilólisis deportiva es una dolencia preocupante. Durante el desarrollo vertebral desde la columna infantil a la columna del final de la adolescencia, la lordosis lumbar aumenta al aumentar la presión posterior. La extensión repetitiva durante este período añade una carga adicional al arco posterior. El niño de 6 años con una simple fractura accidental de la pars interarticularis tiene tiempo para desarrollar mecanismos compensadores estabilizadores gracias a los ligamentos interespinosos y al anillo fibroso. En cambio, en el mundo deportivo, la espondilólisis no consigue la misma compensación y por tanto existe una probabilidad baja pero real de desarrollar una espondilolistesis .
Clasificación de las espondilolistesis
El Dr. Wilte subdivide esta patología en cinco tipos:
Tipo I. Displásica o congénita: se produce por una deficiencia congénita o una alteración en la orientación de las facetas articulares de L5 y S1 que permite un deslizamiento de la quinta vértebra lumbar sobre el sacro. Esta displasia de la facetas junto a alteraciones del raquis como pueden ser vértebras trapezoidales, promontorio sacro en S itálica y espina bífida oculta, facilita estos grandes desplazamientos.
Tipo II. Ístmica o espondilolítica: no hay displasia de las facetas, pero sí un defecto en la pars interarticularis. Sus desplazamientos son discretos. Hay 3 subtipos:
lítica: se encuentra una fractura en la pars.
alongada: hay una elongación de la pars, sin fractura.
traumática: producida por una fractura aguda en la pars.
Tipo III. Degenerativa: el deslizamiento es secundario a una artrosis de las facetas articulares. Se da en la cuarta y quinta vértebras lumbares.
Tipo IV. Traumática: el deslizamiento es ocasionado por una fractura de la vértebra en un sitio diferente a la pars.
Tipo V. Patológica: en este caso hay una enfermedad ósea que afecta a la pars o al pedículo que se debilita y permite el deslizamiento.
En el campo de la medicina deportiva, únicamente los tipos I y II se presentan en niños y adolescentes. En estos casos es muy importante evaluar el grado de displasia que sufre el paciente para conocer el riego de sufrir un mayor o menor desplazamiento.
Para la biomecánica de la columna vertebral son factores de displasia la forma de las facetas articulares, el promontorio sacro, el aspecto trapezoidal del cuerpo vertebral y la existencia de espina bífida oculta.
Diagnosis
– Radiología convencional. Es poco sensible pero muy específica. La mejor proyección es la oblicua, pero el diagnóstico de listesis es en la proyección lateral. Es muy importante hacer un estudio funcional que evidencia la inestabilidad.
– Tomografia axial (TC). Mayor sensibilidad que la radiología convencional, pero es una prueba más específica que sensible. Da información de las características del defecto ístmico.
– Gammagrafia ósea planar (GOP). Es muy sensible pero poco específica.
– Gammagrafia ósea con SPECT. Es un método sensible para detectar una fractura por estrés (espondilolisis) pero se debe confirmar con TC .
– MRI. Permite valorar lesiones asociadas.
Abordaje terapéutico
Según los exámenes complementarios tenemos 3 entidades y por tanto abordajes terapéuticos diferentes.

Diagnóstico diferencial: de otra causa de dolor lumbar
Como ejercicios físicos de higiene lumbar el Dr. William Goers incluye la forma física general, el trabajo de fuerza muscular en el tronco (tanto de la pared anterior del abdomen como la lateral), la flexibilidad (tronco, pelvis y musculatura isquiotibial), la resistencia muscular y la higiene postural.
Influencias del deporte en la columna vertebral
La totalidad de los deportes, incluso los más estáticos, obligan a realizar movimientos de flexo-extensión, inclinación lateral o rotación del tronco de los cuales es responsable la columna vertebral que, en caso de deportes de élite, pueden traducirse en lesiones por sobrecarga básicamente de la zona lumbar y sobre todo en los últimos segmentos de esta zona.
Los doctores Ramón Balius Juli i Ramón Balius Matas del Centro de Medicina Deportiva del Consell Català de l’Esport de la Generalitat de Catalunya han realizado una clasificación de los diferentes deportes como entidades potencialmente patógenas para la columna vertebral en los deportistas del programa de tecnificación deportiva y del ARC que se controlan en este Centro Médico. Esta clasificación se basa en:
– el tipo de movimientos vertebrales que comportan y su carácter simétrico o asimétrico.
– la mayor o menor intensidad de estos movimientos, así como la necesidad intrínseca e
inevitable de alcanzarla
– la necesidad intrínseca e inevitable de fenómenos sobreañadidos de carga o de choque.
Cuando valoremos la repercusión vertebral que producen los deportes es importante tener en cuenta la edad biológica del deportista, pues es bien conocido el carácter evolutivo durante el crecimiento de muchas de estas alteraciones vertebrales.
Clasificacion de Balius
I. Deportes vertebralmente negativos: necesariamente se producen movimientos inevitables de flexo-extensión, inclinación y rotación vertebral de forma continuada y forzada a la máxima intensidad, a los que en algunos deportes se asocian simultáneamente cargas continuas o discontinuas, soportadas, en muchas ocasiones, en mala posición. A pesar de seguir una técnica adecuada, es imposible proteger el raquis y al mismo tiempo obtener un rendimiento deportivo bueno.
* Deportes: gimnasia deportiva masculina y gimnasia rítmica y artística femeninas, judo, lucha libre, lanzamientos de jabalina y de peso, saltos atléticos, natación (estilo mariposa).
II. Deportes vertebralmente negativos en potencia: los movimientos de la columna no deben de ser forzados de forma continua e intrínseca en deportes en los que las cargas, si se realizan correctamente, no se asocian a una mala posición o al movimiento. En estos deportes, una buena preparación gimnástica específica y una técnica adecuada permiten estabilizar y por tanto proteger la columna. Pero en el deporte de élite, dejan de ser vertebralmente negativos en potencia para convertirse en vertebralmente negativos efectivos.
* Deportes: halterofilia, atletismo (carreras de obstáculos, lanzamientos en rotación como martillo y disco), remo, vela, ciclismo, motorismo, equitación, waterpolo, saltos de palanca y trampolín, esquí náutico y alpino, rugby.
III. Deportes vertebralmente indiferentes: los movimientos de la columna no son necesariamente forzados, ni van a cargas o choques y únicamente estos se pueden presentar de forma accidental. Tenemos que tener en cuenta que muchos de estos deportes son asimétricos y antes de contraindicar sistemáticamente un deporte de este tipo, tenemos que estudiar caso por caso ya que una modalidad deportiva de las citadas puede incluso ser favorable en una desviación escoliótica.
* Deportes: carreras, marcha atlética, fútbol, esgrima. hoquei, tenis, tenis de mesa, pelota vasca, golf, tiro con arco.
IV. Deportes vertebralmente positivos: son aquellos que aunque comportan movimientos vertebrales continuados, éstos no son generalmente ni necesariamente forzados, no conllevan la realización de cargas y además desarrollan un componente simultáneo de estiramiento.
* Deportes: básquet, balonmano, voleibol, gimnasia no deportiva, natación (excepto mariposa).
V. Deportes vertebralmente peligrosos: son aquellos en que accidentalmente pueden presentarse lesiones vertebrales agudas como fracturas con o sin lesión medular.
* Deportes: montañismo, espeleología, paracaidismo, vuelo libre, esquí, saltos de palanca y trampolín.

Esto es un error, ya que hay diferentes grados de progresión según el tipo de malformación, de la ubicación del defecto y la edad del paciente. También existen casos en que la malformación produce una discreta deformidad que progresa lentamente y se estabiliza al finalizar el crecimiento por lo que es muy importante determinar con certeza cuál es el riesgo de progresión para así poder intervenir en la historia natural de la enfermedad. Muy interesante. Competí en Judo desde los 15 hasta los 55…y sí algunos teníamos problemas lumbares, por malas técnicas o sobre entrenamiento.
